Napoli picked up the pace again and with four wins on the trot they are now a single point behind AC Milan, which means the reigning champions will go to 2nd spot on the league table if they win here.
It's no accident that the sudden surge of form has coincided with the return of Kevin De Bruyne. He's the guy who's unlocking defences - which is one thing that Napoli has a huge problem with.
What this means in practice is that Napoli is just a bit more likely to win a match that they would otherwise stall in and fail to score the winning goal.

This match perhaps would have mattered if Inter spilled some points yesterday against Roma, but they did not. It was 5-2 for the likely 2025/26 Italian champion, and now the difference is 9 points after it was 6 prior to the start of this round.
In other words, Milan must beat the champion Napoli on the road, in Naples, just to keep up with what Inter is doing. That's a tall order.
That being said, Milan has actually done quite well in derbies against Napoli, and in derbies this season in general, and this is in fact the main thing that's keeping their title aspirations alive. Whenever they lose points to a relegation candidate, they go on and win a derby.
Not in Naples though, where they've lost 2-1 in March 2025, so, last season, and they haven't played there since. The last previous result from Naples is a 2-2 from 2023.

Rrahmani and Di Lorenzo are big defensive absences for Napoli, and Romelu Lukaku has fallen out with the club and is presently in Belgium.
Leao trained with the team and he'll either be on the bench or in the starting lineup. Pulisic is a candidate too. Meaning, we're not 100% certain the standard forward duo will run out from the 1st minute tonight. Gabbia is out and Loftus-Cheek returns.

The Betting Verdict
Milan's luck could run out here. As good as they are in derbies, and as good as Luka Modric will be to keep the team calm and the midfield ticking in this heated atmosphere, with KDB back Napoli is simply a better team.
Milan managed to win 2-1 in Milano, but it was a shock when it happened -- it would be a mistake to look at that and think "Milan always beats Napoli at home".
They did it that time despite getting a red card in the 57th minute, and Napoli posted an almost 3.00 xG while Milan had 0 attempts on goal following that red card. Can't judge a book by its cover.
The point is, Milan has to outperform themselves to beat Napoli. Will this happen in Naples? Possibly. But with Nkunku and Fullkrug announced in the starting lineup, it's not very likely.
