Despite being on 15 points, same as Inter and a point behind AC Milan, Napoli isn't doing all that well. They were shown their limits several times already - first against Manchester City, then against AC Milan, and finally against PSV as they lost 6-2 on Tuesday. That's six goals conceded against a second-tier European side.
At the same time, Napoli continued to struggle as a favourite. Even though they won all these matches, they often had to do some chasing. 3-2 against Pisa and 2-1 against Genoa were hardly dominant victories. They were difficult ones.
What all this means is that Napoli seems to run out of options whenever the going gets tough, even though they usually find a way to beat the opponents they realistically must beat nine times out of ten. The biggest cause for concern here is that Napoli doesn't even seem to have a functional game plan for any situation in which they're not favoured to win. At least they haven't showed it yet.
Also note that this team is still deprived of Romelu Lukaku, the one-man offense, and has to make do with Rasmus Hojlund. While a quality forward, Hojlund can't carry the team on his own.
Inter Milan Team News
Inter continued their win-to-nil run in the Champions League. Ajax, Slavia and Royale Union fell 2-0, 3-0 and 4-0 respectively and this means Inter is at the top of the table together with PSG, Bayern, Arsenal and Real. But we always knew Inter is a flat-track bully; the true tests will come against Liverpool in December and against Arsenal and Dortmund in January.
In the domestic league, Inter now has an unpleasant task of having to keep up with the table leaders AC Milan, as the Rossoneri will probably win their fixture against Pisa on Friday. If Inter doesn't win here then they'll find themselves three or four points behind Milan this early into the season. Also, Napoli is on 15 points, same as Inter, so in case of defeat Napoli solves all its problems and goes up.
That being said, Inter is known to step up to the occasion and win against a big team when the going gets tough, as this is a powerful squad without any big holes. All other sides have a weakness, Inter simply does not. The one thing to keep an eye on is whether Marcus Thuram is available to play alongside Lautaro Martinez, as this side is far less dangerous when one member of this duo is not on the pitch.

Inter (3-5-2): Sommer - Akanji, Acerbi, Bastoni - Dumfries, Barella, Calhanoglu, Mkhitaryan, Dimarco - Lautaro, Bonny. Coach Chivu.
Thuram is still out for Inter and he should be back for the next round. Lobotka is also still out for Napoli so Billy Gilmour will deputise at the holding midfielder position.

The bookmakers are obviously expecing a 1-1 here as the price on Under 2.5 Goals is 1.73 while the price on Both Teams To Score: YES is 1.80. That being said, The Draw is priced at tempting 3.25 as Inter is favoured at 2.35 because they always do well in derbies against all other big teams in Italy.
The Betting Verdict
While we can certainly value Napoli and Inter according to their current roster and form, the H2H record should reveal some bigger clues on how these teams do when they face each other. In eight out of ten most recent meetings, both teams scored, and in seven out of ten it was a low-scoring game.
This is no accident -- both sides are approaching this fixture as it will be a trench warfare, and then it is. Only sporadically will it evolve into a four-goal thriller, but as a general rule of thumb the teams will be cautious and will look to close down space and tackle hard.
In addition to the above, both sides were involved in Champions League fixtures on the road, and tired legs usually mean there is less creativity than usual in the offensive third. This game looks like an under. Also consider a bet on Inter.
